The title of the article I chose was Overprescribing and Inappropriate antibiotic selection for children with pharyngitis. This article was published in JAMA pediatrics in 2014 and was written by Shapiro et al. Th articles is about a cohort study that analyzed data from the National Ambulatory Medical Care survey and the national hospital ambulatory medical care survey. The study found that over 11 million pediatric visits for pharyngitis occurred annually from 1997 to 2010. Furthermore, 60 percent of those visits received antibiotics despite only 37 percent of them being bacterial in nature. The study also concluded that narrow spectrum antibiotics such as penicillin is under prescribed when compared to broader spectrum antibiotics in the setting of pharyngitis. Some of the limitations this article faced include the fact the frequency of strep testing and prevalence of penicillin allergy were not taken into consideration. In addition, the accuracy and specificity of the ICD codes were uncertain. In conclusion there is an apparent gap between the observed practices in this article and the guideline recommendations within regard to treatment of pharyngitis in pediatrics. Targeted education and provider audit feedback can be used to limit over prescribing and reduce prescription of broader spectrum antibiotic in the face of pharyngitis.
