I really enjoyed my internal medicine rotation; I feel as if it was an amazing teaching environment, and I was able to learn a lot. At this rotation I sharpened many different skills such as taking an ABG, writing a plan, writing an assessment, and how to efficiently hand off a patient to another provider. During my mid site evaluation, I presented an 80-year-old male who presented complaining of left sided chest pain that had been going on for 3 days. When I was presenting this case, I felt very comfortable because I had interviewed the patient thoroughly and went over the case with one of the PAs in the internal medicine department. During my presentation I outlined different tests I would order or that were already ordered such as a CBC, BMP, cardiac enzymes, and d-dimer. The name of the article I chose was “Association Between Intensity of Low-Density Lipoprotein Cholesterol Reduction With Statin-Based Therapies and Secondary Stroke Prevention” by Meng Lee et al. The article was published in the JAMA neurology in Feb of 2022. This article was Meta-analysis of Randomized Clinical Trials that utilized 37 full articles for detailed assessment, out of those 26 did not meet the inclusion criteria; therefore, the final analysis was composed of 11 randomized clinical trials and 20 163 patients with stroke were enrolled. Chief Complaint: Difficulty speaking x 3 days 60 y/o male with a history of HTN, HLD, current smoker (20 pack year), and stroke on 01/23 (no residual deficits) presents to the ED sent by his neurologist due to dysarthria, slurred speech, and numbness for the past 3 days. My surgery rotation was very interesting, I was exposed to a lot of different things and people. My first time scrubbing into a surgery I was terrified but as I began to get more comfortable after I scrubbed into more surgeries my confidence increased. I really enjoyed going to the different clinics and conversing with and examining patients. I also enjoyed the time I spent with the PA’s who were working the floor and ICU. During my mid site evaluation, I presented a 24-year-old female who was presented to the general surgery clinic complaining of consistent abdominal pain suspicious of cholecystitis, that had been going on for 1 month. When I was presenting this case, I felt very comfortable because I had interviewed the patient thoroughly and I felt as if I knew gastrointestinal issues well after my emergency medicine rotation.
